Benicia Unified School District

In compliance with federal law, Benicia USD will provide food substitutions to a child whose allergies, intolerance or disability restricts their diet.

The Medical Statement to Request Special Meals and/or Accommodations Form is attached.

The form is to be completed and signed by a State Licensed Healthcare Professional.

Once the completed Medical Statement is received, a special custom designed menu is created for the student. The information is relayed to the Central Kitchen who prepares the special menu for the student.
Please allow up to 5 days for the menu to be created and meals to be delivered to the student.
